Option 1: One visit per week (60–90 minutes) | $600 per month — month-to-month
Option 2: Two visits per week (60–90 minutes) | $800 per month — three-month commitment required

The Sanctuary at Home Care Plan is designed to create a space of peace, safety, and holistic support — where care and comfort meet right where you live. This plan nurtures body, mind, and spirit while promoting dignity, independence, and emotional well-being. It’s ideal for individuals managing chronic conditions, recovering from illness or surgery, or simply seeking to maintain a stable, supported lifestyle.

Each visit includes a detailed medical history review, ongoing health assessment, and vital signs monitoring, along with medication management — including weekly or monthly pill-dispenser preparation, medication education, and coordination with healthcare providers. You’ll receive personalized health education, lifestyle guidance, and a collaborative care plan to improve daily quality of life and strengthen long-term outcomes.

The Sanctuary at Home plan also encompasses:

• Assistance with activities of daily living (ADLs)

• Home safety assessments with personalized recommendations

• Nutritional and diet guidance for sustained wellness

• Monitoring of ongoing health concerns such as diabetes, heart disease, or post-surgical recovery

• Coordination of appointments and provider communication

• Optional accompanied visits to doctor appointments or local errands (available for an additional fee)
