🌿 Restorative & Preventive Care Plan
One-on-one personalized visits | 60–90 minutes | One visit weekly for six weeks | $600 per six-week interval
The Restorative & Preventive Care Plan is designed for individuals transitioning from rehabilitation, a hospital stay, or recovering from illness, injury, or age-related decline. This plan provides a proactive, hands-on approach to help restore and maintain the highest possible level of function and independence — all within the comfort and safety of home.
Each visit combines traditional prevention and restorative strategies with personalized education and goal setting to promote long-term health and stability. Care is focused on evaluating mobility, monitoring physical progress, and assisting with activities of daily living (ADLs) and self-care tasks — always with a strong emphasis on home safety and independence.
Clients receive a comprehensive health and medical assessment, including a detailed medical history review and head-to-toe evaluation. The care plan includes nutrition guidance, fitness and mobility support, stress and sleep optimization, and education for both patient and family.
