🌿 Palliative Care Plan
Two to five visits per week | 60–120 minutes per visit | $700–$1,200 per four-week cycle (price varies by frequency and visit length)
The Palliative Care Plan offers comprehensive nursing for clients living with serious or advanced illness who wish to receive attentive, professional support at home. This plan focuses on improving quality of life, reducing discomfort, and supporting both patient and family—physically, emotionally, and spiritually.
Each plan is customized to the client’s level of need, ranging from two to five visits per week (60–120 minutes each). Care includes:
Pain and symptom management – monitoring, medication administration, and non-pharmacologic comfort measures.
Medication and equipment coordination – communication with providers and pharmacies, assistance with medical devices such as oxygen or feeding tubes, and recommendations for supportive home equipment.
Comprehensive assessment and ongoing evaluation – head-to-toe review, wound and skin care, and continual reassessment of pain, mood, and physical function.
Emotional and spiritual support – compassionate presence, listening, and guidance to reduce anxiety and depression, promoting a calm environment for patients and families.
Family education and advocacy – helping loved ones understand changes, manage caregiving responsibilities, and access community or hospice resources when appropriate.
Care-plan adjustments – regular communication with physicians and specialists to update goals of care as the illness progresses.
Optional 24-hour tele-nursing access is available for urgent questions or after-hours guidance at a premium rate. Clients may also request extended in-person visits or overnight monitoring for additional comfort and continuity.
