Two visits — 60 to 90 minutes each, three weeks apart | $180 total

The Assessment Care Plan is designed for individuals who wish to take a proactive approach to their health and well-being. Whether you’re managing minor age-related changes, experiencing mild aches, or simply wanting to stay on top of your overall wellness, this plan provides a private and personalized health evaluation in the comfort of your home.

Each session includes a comprehensive head-to-toe nursing assessment, detailed medical history review, and evaluation of current medications and lifestyle habits. Together, we’ll discuss your goals, identify potential areas of concern, and create a collaborative care regimen to support your continued well-being. You’ll also receive education for prevention, vital signs monitoring, and gentle guidance for lifestyle moderation to enhance daily living.

Following completion of the two-visit assessment plan, clients may choose to continue their wellness journey through a Quarterly Wellness Check — one visit every three months for $480 per year (90 minutes per session). This ongoing option helps maintain stability, encourage accountability, and ensure your care plan evolves as you do.
